CLN: references/tests for item_cache

test_to_dict_of_blocks_item_cache is about _item_cache invalidation, but IIRC we got rid of that cache a while back.  grepping for "item_cache" i see a bunch of comments that are no longer accurate and tests that are no longer testing anything.  These can be updated/removed.
